Report Guidance
Have questions about the reports required by your Title V Operating Permit? Check out the guidance on these reports (including sample compliance certification, monitoring, and deviation reports).
The following administrative changes have been made to language in Section I, General Provisions of Title V Operating Permits. The changes are listed here for informational purposes. If you have a question on any of these issues, please contact the Operating Permit section at (801)536-4000.
- 1997 Language for Compliance Certification Provision I.L
- Inventory Rule Changes in Provision I.U
- Inventory Rule Renumbering
- New (2004) Language for Compliance Certification Provision I.L
- New EPA Address Change
Revisions to Compliance Certification Provision I.L (8/3/2004)
Revisions to UAC R307-415, the rules governing the Operating Permit Program, became final on August 3, 2004. The changes incorporate language promulgated for 40 CFR Part 70 by EPA on June 27, 2003. The changes require sources to certify whether compliance with each permit term was continuous or intermittent, instead of whether the monitoring method was continuous or intermittent. The preamble to the Federal Register notice contains EPA information on making this determination.
Some of the revisions to the rules affect the wording in Section I of your permit. The modified rules immediately take precedence over the affected permit language. The new language for provision I.L of your permit is available.
Inventory Requirements Change (12/31/03)
Provision I.U of Title V permits lists inventory requirements. As of 12/31/2003, inventory requirements have been changed. R307-150 now encompasses all inventory requirements. The HAP inventory requirements of R307-155 and the Emission Statement requirements of R307-158 have been repealed. As a result, in existing permits, ONLY Provision I.U.1 is effective. Provision I.U.2 and, if listed, Provision I.U.3 are no longer in effect.
Permits issued after 1/1/04 will contain only the effective term as Provision I.U.
EPA Address Change (Spring 2007)
The new address for reports, notifications, or other correspondence related to permit modifications, applications, etc.in Provision I.S.3.b is:

The address for compliance certifications in Provisions I.L.2 and I.S.3.b is also changing in the same manner. The correct address for these submissions is:

Environmental Justice (Mail Code 8ENF)
1595 Wynkoop Street

Permits issued after Spring 2007 will contain the correct address. Please make a note of this change in your existing permits.
Revisions to Compliance Certification Provision I.L (8/6/99)
Revisions to UAC R307-415, the rules governing the Operating Permit Program, became final on July 15, 1999. The changes incorporate new language promulgated for 40 CFR Part 70 in conjunction with the rulemaking for the Compliance Assurance Monitoring (CAM) rule. Some of the revisions to the rules affect the wording in Section I of your permit. The modified rules immediately take precedence over the affected permit language. The new language for provision I.L of your permit is available.
Inventory Requirements Rule Number Change (4/15/99)
Provision I.U of Title V permits has been modified to reflect rule number changes made in March of 1999. Permits issued after 4/19/99 have the correct rule numbers in them already. The changes are as follows:
- In Provision I.U.1, R307-155-1 was changed to R307-150.
- In Provision I.U.2, R307-155-2 was changed to R307-155.
