Guidance for Reports Required

by Your Title V Operating Permit

 

Sources with Title V Operating Permits must submit several reports to DAQ each year. These include the annual compliance certification and at least two six-month monitoring reports. Additional reports may be required in specific conditions in Section II of the permit.

The annual compliance certification is the cornerstone of Title V and is required by condition I.L of the operating permit. In this report, the source must certify compliance with the terms and conditions contained in the permit, including emission limitations, standards, or work practices (see UAC R307-415-6c(5)). There are conditions in Section I, General Provisions, that must be certified along with the specific conditions in Section II (and in Section IV, for Acid Rain program sources). In order to help sources correctly complete the certification, a sample compliance certificate is available.

A monitoring report is also due from every source with a Title V operating permit at least once every six months. The content of this report depends on the reporting requirements in the permit; at a minimum, it should contain a list of all permit deviations for the six month period covered by the report. (Recall that, for Utah operating permits, a deviation refers to any failure to comply with the terms of the permit, including any failure to perform the specified monitoring, recordkeeping, or reporting.) If no deviations have occurred, the monitoring report may be as simple as a statement to that effect. Sample deviation and monitoring reports are available.

It is critical that a responsible official certify all reports in accordance with condition I.K of the permit.
